Directory Update


Current and new CHCI members and new members may use the form below to submit changes to existing records in the CHCI online members directory, or to create a new record. Please be sure to indicate whether your submission is a new entry or an update. For updates, you need only include new or updated information. If you have any questions about this form, please contact us at CHCI@duke.edu. After submitting your form, a confirmation screen will appear.

Changes will be entered into the CHCI database, listserv, and online directory. We strive to make all changes within 48 hours of receipt, and a CHCI staff person will contact you directly to inform you that the change has been made.
